A signed form needs to be filed with the MVA prior to playing in a tournment.  The preferred methods of delivering the signed form are mail, fax, or scan and send as an email attachment to rick@mnvba.com.

$30 may be paid by check and mailed with the form

$40 may be paid at the tournament site

A signed copy of your MVA Individual Member registration is required to complete your registration. You may either email or fax (715-778-4249)  the form or bring it the morning of your first tournament and submit it before you play. If you submit the form at your first tournament please bring a copy of your Pay Pal payment verification.
