The MVA does not currently accept faxed entry forms. Credit cards are only accepted online.
MVA policy is that entry fees will not be refunded for teams that drop from tournaments for any reason past the deadline date. The MVA is billed and responsible for all courts reserved up to a month prior to all tournaments. Unless the MVA charges a higher fee overall for tournaments during the season to cover for teams that drop from tournaments or site refund funds for the same reason, it is not possible to refund entry fees.
